Before your planning meeting, it’s worth noting the reasons why you feel you would like to have Support Coordination in your NDIS plan.
We've outlined a range of reasons why some people may seek the assistance of a support coordinator. If any of these reasons are relevant to you, you may want to highlight them with your Planner or Local Area Coordination (LAC) when considering the inclusion of a support coordinator.

Asking for support coordination
Your best chance of having support coordination included in your NDIS plan is to highlight the reasons why you want it. Here are some examples.
For example, because of:
- communication difficulties
- changing or high health needs
- complex support needs
- multiple disabilities
Your social situation / support network
For example, because:
- there are other family members with a disability
- your family situation
- little or no support networks
- your current living arrangements
Your limited experience exercising Choice and Control
For example, because you need assistance with making informed choices and having control over how your NDIS plan is implemented. Including knowing how to:
- select providers
- provide feedback to your providers
- be an active partner in your therapy
- provide compliments complaints and feedback to your provider
- understand and unpack your budget
Your personal situation
For example, because you:
- have cultural and language barriers
- are new to NDIS and its processes
- are a younger person in an Aged care facility
Discuss at your planning meeting
If you think you would benefit from Support Coordination, you will need to discuss what level of Support Coordination you need at your planning meeting.
These are the codes you will see if Support Coordination has been included in your NDIS plan:

||Assistance to strengthen participant’s abilities to coordinate supports & participate in the community. This includes resolving points of crisis, developing capacity & resilience in a participant's network & coordinating supports from a range of sources.
|Coordination of supports
||Further qualifications/experience required to strengthen a higher needs participant’s ability to coordinate their supports & participate in the community. This may include resolving points of crisis & developing resilience in the participant's network.
|Specialist support coordination
||Time limited support coordination, within specialist framework necessitated by high risks in participant's situation, to address barriers and reduce complexity in environment, assisting to connect with supports, build capacity and resilience.
Is Support Coordination already in your NDIS plan?
If you have a plan where Support Coordination is included, your Planner or LAC will provide you with the opportunity to nominate a provider. At this point you can select Integra as your support coordination service provider. A plan handover is usually arranged between the planner and support coordinator. We will also be in touch with you to discuss your needs and next steps.
